It is very easy to do. Just enclose a check made out to “CAMLT Lab-PAC” and complete the information form below. This information must now be kept for all political contributions. Then, return the check and form to the CAMLT Executive Office, 1895 Mowry Ave., Ste. 112, Fremont, CA 94538. That is all you have to do!
